Reserve Request Form

Use this form to place material on Reserve at the Circulation desk of Cochran Library. If you own the
material you would like to place on reserve, please send it to Shirley Reid at the Library via campus
mail within 24 hours after you send this form. If the Library owns the material, the Library staff will
pull it for you.

To avoid any delay in processing, please fill out this form completely.
